Function / Content
When the Registrar of Births, Deaths and Marriages registered a death, a 'registration of death' form was completed and forwarded to the undertaker responsible for burying the body. This was a requirement of section 17 of the Registration of Births, Deaths and Marriages Acts 1915 and 1928. The format of the form was prescribed in the seventh schedule to those Acts.
After the burial the undertaker completed a certificate of burial and returned it to the Registrar as required by section 18 of the two Acts. The format of the certificate was prescribed in schedule nine to the two Acts.
Recordkeeping System
The two forms were printed on one perforated sheet. The Registrar inserted the details in the 'registration of death' part of the form, and, after the burial, the undertaker completed the 'certificate of burial' part of the form and returned it to the Registrar.
This series comprises the Registrar's 'registrations of death'. It was apparently compiled by the Oakleigh Pioneer Memorial Park Advisory Committee. The date of compilation is not known.
